You Can't Escape the Paperwork:

The good news is, we keep this to a minimum, and you only have to fill it out and bring it along the first time you volunteer (unless your information changes); the bad news is, you have to do a little paperwork so that we can keep you safe and sound on the work site. 

Print, Sign, and Turn in to the Site Supervisor:

Release and Waiver of Liability and the Emergency Contact Form (For Volunteers 18 years old or better)

Parental Permission Form and Release and Waiver of Liability for Minors and the Emergency Contact Form (For Volunteers age 16-18)
